Make an appointment on Calendly!
- Pick your campus and select from the available dates and times. If you can’t make it at one of those times, still make the appointment and then contact us at Loaner.Equipment@centralaz.edu to set up an alternate pickup time. If you miss your appointment and don’t contact us we can only hold the equipment until the end of the day of your appointment.
- You must be attending classes during the semester you wish to sign out equipment for, please bring a copy of your schedule along with you when you come to pick up your equipment.
- Print out and read the Equipment Use Agreement and bring it with you to your appointment. Make sure to provide your personal e-mail and phone number. The contract is available here. Equipment Use Agreement Form (PDF)
- If you are under 18, a Parent or Guardian will need to come to the appointment with you and sign the contract as well.
- You must bring either your Student ID or a Photo ID such as a Driver’s License or other State-issued photo ID to your appointment.
