Distance Learning – Part Time – Complete in One Year
The Community Health Worker (CHW) Certificate Program provides an opportunity for individuals to develop skills as a frontline public health worker. This worker is a trusted member and has a close understanding of the community they serve. This relationship enables the CHW to serve as a liaison between health services and the community. A CHW builds health understanding and self-sufficiency in individuals within the community through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.

CPH 160 Introduction to Community Health Worker (3 credits)
CPH 200 Community and Public Health (3 credits)
CPH 178 Personal Health and Community Wellness (3 credits)
CPH 185 Health Communication (3 credits)
HCC 103 Introduction to Medical Terminology (1 credit)
CPH 180 Chronic Disease Management (3 credits)
CPH 196 Community Health Worker Internship – 90 hours (2 credits)
Total Credits: 18 – Tuition $1,548 * (Based on CAC in-state tuition of $86.00 per credit hour). Does not include cost of textbooks or other requirements.

Excellent communication and written skills. Willingness to help individuals and communities in a variety of areas of community health and public health, including but not limited to: health promotion, disease prevention, wellness, and chronic disease management.
Acceptance into the program required; HS diploma or GED recommended. CAC does require that new students successfully pass placement tests prior to enrolling in classes. For more information, go to www.centralaz.edu/admissions/placement/.
Students must live in the State of Arizona and must complete a 90-hour internship. Prior to beginning CPH 196, students must submit to a background check, drug screen, TB skin test and have up to date immunizations (completed at student’s expense).
Students in the CHW Certification Program are eligible to apply for Federal Financial Aid. Also, Maricopa County Department of Public Health has scholarships available to cover the tuition and textbook for five students.
