Distance Learning - Part Time - Complete in One Year

The Community Health Worker Certificate provides the opportunity for individuals to develop skills as a front line public health worker who is a trusted member of and/or has a close understanding of the community they serve. This relationship enables the CHW to serve as a liaison between health services and the community. A CHW builds health understanding and self-sufficiency in individuals within the community through a range of activities such as: outreach, community education, informal counseling, social support and advocacy.

Recommended Proficiencies
Good communication and written skills. Willingness to help individuals and communities in a variety of areas of public health, including but not limited to: disease prevention, wellness and chronic disease management.

Prerequisites
Acceptance into the program required; HS diploma or GED recommended.

Program Requirements
Students must live in the State of Arizona and must complete 90-hour internship. Prior to beginning CPH 196, students must submit to a background check, drug scree, TB skin test and have up to date immunizations (completed at student's expense).

Application Deadline
Applications are currently being excepted for the 2017-2018 CAC Community Health Worker Certificate Program.
Deadline for applications is July 1, 2017.
