|SOS Referral Form|
Supporting our Students (SOS)
The Office of Student Retention in conjunction with various student and academic resource areas would like to introduce the second phase of our Early Alert Program, Supporting our Students (SOS). SOS is a program designed to identify students who are having academic difficulty at any time throughout the semester. The online form allows instructors (full and part-time) and staff to alert the Office of Student Retention of any student who is not making satisfactory progress. Once a student is identified via the online form, the student will be sent appropriate resources. In addition, the Office of Student Retention or the appropriate department will attempt to contact the student and connect them to the appropriate resources in an effort to improve the student's academic performance. The faculty/staff member making the referral will also be provided follow up information.
Examples of concerns that can be addressed through SOS are:
• Undecided career goals - lack of direction
• Test Anxiety
• Poor Attendance
• Time Management Skills
• Sudden or unexpected drop in grades
• Personal Issue(s) affecting the student (For emergency, health, and/or safety concerns please contact Campus Police or your local police department.)
• Tutoring Recommendation
Please note, this form is not to be used in lieu of CAC's Incident Report (IR), which is for documenting when students exhibit concerning behaviors outside those that can be addressed through normal classroom management.
